Origin of the IPSS Metric
The International Prostate Symptom Score (IPSS) was originally created by the American Urological Association (AUA) and adopted worldwide by the World Health Organization (WHO) and the British Association of Urological Surgeons (BAUS). It serves as the clinical benchmark for quantifying symptoms related to benign prostatic hyperplasia (BPH).
The Seven Clinical Dimensions of Lower Urinary Symptoms
The complete questionnaire evaluates seven specific symptom areas experienced over the past month:
- Incomplete Emptying: The sensation of retained volume immediately following voiding.
- Frequency: Having to pass urine multiple times within 2-hour intervals.
- Intermittency: Flow stopping and starting spontaneously during a single void.
- Urgency: Sudden, uncontrollable urge that is hard to defer.
- Weak Stream: Reduced velocity and force of the urinary jet.
- Straining: Needing to push or strain abdominal muscles to initiate urination.
- Nocturia: Number of episodes of awakening from sleep to void.
Interpretation of Symptom Tiers
| Total Score | Classification | Standard Clinical Guidance |
|---|---|---|
| 0 – 7 | Mild Symptoms | Watchful waiting, lifestyle guidance, botanical nutritional support. |
| 8 – 19 | Moderate Symptoms | Active lifestyle interventions, standardized phytotherapy, baseline GP review. |
| 20 – 35 | Severe Symptoms | Formal GP / Urologist consultation, flowmetry, potential pharmacological or surgical therapy. |
